According to a report of the accident by Deputy Carl Wade, he was dispatched to be on the lookout for a blue Toyota that was failing to maintain its lane. The deputy said, soon after that dispatch, he was sent to the scene of the accident just north of Pioneer Road involving the vehicle.
Wade said, when he arrived at the scene, medical personnel were already tending to the driver of the car, who was later transported to the Medical Center of Central Georgia.
The deputy interviewed four witnesses to the accident and said he received similar descriptions of what happened. From the interviews, he determined that Vaughn was having a difficult time staying in his lane and seemed to suddenly pick up speed. At that point he went off the roadway.
The first person on the scene reported that Vaughn was unresponsive and asked responding units to hurry.
