Daily Grace Adult Day Health Services is honored to partner with the Veterans Administration through the new Community Care Network (CCN).
Feb. 13, the Community Care Network offered by the Veterans Administration opened in the Southeast Region. Daily Grace Adult Day Health Services Center is one of the partners to offer out-of-home long term care for eligible Veterans. A physician at the Veterans Administration Medical Center will determine if they are eligible for the Community Care Network.
The Community Care Network was developed from the VA MISSION ACT that was signed into legislation June 6, 2018. It allows more choices and provides robust care coordination using one consolidated program instead of multiple programs. This is a direct link with the VA to connect veterans with high qualified community providers to ensure veterans receive timely and high-quality care that they deserve.
Community Care providers also include medical, dental, surgical, pharmacy, and D.M.E. services.
The criteria for Adult Day Health services are the individual must have ongoing health conditions, difficulty with mobility and/or require assistance for self-care tasks, or cognitive disability. We accept clients who need the use of a cane or walker for mobility.
The benefits of our services will make a difference in the lives of homebound adults and seniors who are at risk for permanent nursing home placement.
Experienced nurses and therapists can make the difference of your loved one to improve their health, mobility, mood, and cognitive function. Caregivers can be assured that their loved one will be taken care ofby qualified healthcare professionals so they can have respite time to take care of themselves or do what they would enjoy doing.
